spectacular misses

I am hoping if anyone have solutions, drills, swing thoughts, etc. to help me with not missing shots spectacularly.

My spectacular misses happens when one or more of the following occurs:
1) My head comes up and/or turn left (I'm a right-handed golfer).
2) My right elbow works behind me instead of side or in front of me, forcing me to "push" my hands out to hit the ball.
3) My right/hip goes up instead of around at transition, resulting in 1 and 2.
4) I lose my spine angle; it rounds and/or goes more vertical than what I had it at address.

I hit a lot of good to average shots which leads to pars and birdies during a round, but I also have 7-8 of spectacular misses which leads to bogeys and double-bogeys.
